Do all insulin pumps check blood glucose levels?

I (hopefully) will be getting an insulin pump on Monday (yay!) and was wondering if all models automatically check blood sugar levels every 5 minutes or whatever, or if that’s not standard. Might I still have to stick my finger to check my levels every day?

Unfortunately, dear lady, pumps by themselves don’t test your blood glucose levels. That only happens if you have them attached to a CGMS (Continuous Glucose Monitoring System), and they are very expensive. (It’s another attachment that needs to be inserted subcutaneously, and needs to be changed every few days. I’m not sure how expensive they work out at in the United States, but when I was using them, over here in the UK, they cost approximately £50 each. At today’s exchange rate, that’s got to be in excess of $70 each.)

Do you want some more bad news? I’m afraid, whilst the pump is new to you, you’re going to have to test your blood glucose levels more frequently. This is because pumps only use fast-acting insulins, and there’s a greater danger of you having hypoglycaemic [hypoglycemic] attacks. It would also be safer for you to purposely wake yourself up during the night to test, at least until you’re sure that your blood glucose isn’t dropping unexpectedly.

I know it sounds like I’m painting a black picture of pump use, but really I’m not. In fact, if anyone were to try taking my pump away from me I’d fight them to the death … and I’ve been using one for approximately 12 years now.

I’m sure that once you’re used to your new pump you’ll find it such a wonderfully liberating thing to have.

Wound care, healing wounds, diabetic ulcers: High Power Laser Therapy a new therapeutic approach

Our office uses a multifaceted approach to address non-medical peripheral neuropathy. This includes many patients who have not been able to take standard drugs such as Neurontin neuropathy, Lyrica and Cymbalta due to side effects. In addition we see patients who initially were managed with However these drugs, the treatment dose was increased to the maximum and the patient continues to have symptoms more. As such we see our share of diabetics and patients with kidney disease and diabetic ulcers wound care issues. Many are the treatment of diabetic ulcers and wounds slow to heal the wound care centers that may include hyperbaric oxygen therapy. They are however aware that the laser therapy has been show faster growth and tissue repair, reduce inflammation, improve vascular activity, reduce tissue scar and improve wound healing by stimulating the development of fibroblasts and collagen production. Here are two recent articles in the literature science on laser treatment and wound healing.

The modulation of collagen fibers during wound healing of experimental skin was studied in Wistar rats treated photobiomodulation 112 laser. A standardized injury 8mm in diameter was made on the dorsal skin of all animals. In half of them, 0.2 ml of silica suspension was injected along the border of the wound to improve collagen deposition and facilitate compliance. The others received saline as vehicle. The treatment was performed using ray laser with semiconductor aluminum gallium-arsenide diode 9mW applied every two days (total dose = 4J/cm2) on the edges of the wound. Cuts tissues obtained from four experimental groups representing sham-irradiated animals, laser, silica and the combination of both, have been studied after 3, 7, 10, 15, 20, 30 and 60 days from the application of the laser. The area of damaged skin was surgically removed and subjected to histological immunohistochemical, ultrastructural and immunofluorescence studies. Besides the degree and the arrangement of collagen fibers and their isotypes, the degree edema, the presence of several cell types especially pericytes and myofibroblasts, have been described and evaluated. Observation Sirius-red stained slides under polarized microscopy revealed to be a great help when analyzing morphological changes of collagen dynamic. It has been shown that the laser application was responsible for the regression of edema and decreased number of inflammatory cells (P <0.05). A clear increase in the number of actin-positive cells was observed in the laser-treated wounds. collagen deposition was lower than expected in the wounds treated silica, and laser treatment has contributed to its better differentiation and modulation in all groups irradiated. Thus photobiomodulation laser was able to induce several changes in the skin healing process, particularly in promoting fiber collagen newly trained to be better organized and compactedly eliminated.

The aim of the study is to evaluate effectiveness of laser therapy low level for the healing of wounds in combination with the prevention and Extendicare Wound Management Program. Sixteen people at a Canadian Extendicare nursing home had a total of 27 sites treated consisting of 23 open wounds and 4 'at risk' areas. Of the 23 wounds open wounds between the two toes are not able to be "tracked" and considered "immeasurable 'wounds, resulting in 21 open, measured injuries. The four "at risk" (closed) areas were treated prophylactically. pressure ulcers, venous insufficiency and diabetes were included. The majority (12/21) or 57.1% of injuries were chronic (> or = 3 months duration) and 42.9% were acute (<3 months). The main outcome measures included the PUSH Tool score, EZ traced graphics and photographs. Secondary outcome measures were used to better understand potential barriers to successful integration into clinical practice. Reaction on the effectiveness of laser therapy low level, education program and determining the relevance of hands was requested by staff. At the end of nine weeks of trial, the majority (61.9%) of the 21 wounds achieved significant improvement (> or = 50% wound closure). Nine (42.8%) had 100% closure. Some improvement was observed in 14.3% and 23.8% of injuries showed no change. Chronic and acute wounds had similar improvement. No injuries in this weakened, frail population deteriorated during the study and no adverse consequence of treatment were encountered. Without the support staff, although new technologies have positive clinical outcomes, the success would be limited. Staff rated low level laser, easy to learn and use, effective for the majority of their inhabitants value overtime. Staff requested a continuation of weak laser level even after completion of study.
